Joint infection: Studies show septic arthritis causes swelling warmth pain tenderness, IMO the most important cause to rule out. Also, gout, pseudogout, ligament tear if there has been trauma, meniscus tear or other pathology, distal femoral defect, osteoarthritis. Without any more info, best to advise a full history and physical.
