Go to emergency room: Any time there is concern for feces in the vomitus, a small bowel obstruction needs to be ruled out. An abdominal x-ray is required. Rehydration and medication for nausea is also available.

Not a good sign: Generally, vomit that is feculant (smells like stool), is indicative of a distal bowel obstruction. You must take him to the hospital where further testing will be done to rule this in or out. Seriously.
