Persistent back pain: without explanation should always be checked for blood clots in the lung or disc disease. As a smoker, cancer should always be a possibility in the back of your mind. Coupled with the dry persistent cough, you really need to have a healthcare provider give you X-rays of both your lungs and your spine to rule out mass or COPD, then get a contrast CT of the lungs to rule out blood clot.
