IIH: Idiopathic intracranial hypertension (IIH) can lead to spontaneous CSF leaks, and these generally do not stop on their own (the IIH keeps up the pressure and cause the leak to continue). So surgery is usually required to avoid other complications of ongoing csf leak (like meningitis).
