Maybe pilonidal cyst: A pilonidal cyst is often the culprit of these symptoms. There is a cystic area under the skin that often becomes infected and drains spontaneously. They can be very painful. He should see his primary care doctor for referral to a surgeon for excision of the cyst, which should take care of the problem.
