Try using flushable: wipes rather than toilet paper. There are many different types available. Some made specifically for hemorrhoid patients and other just general wipes. If the problem is not any better, you should see a health care provider just to make sure that this is a hemorrhoids problem and not something else.

Use topicals: There are special lotions sold over the counter for comfort in the perineal area. They contain a mild pain reliever as well. Never use dry toilet paper, and try dabbing vs wiping. The skin breaks sound like a reaction to hard dry stools. Drink more water and use agents like Miralax to soften the stool
