Many possibilities: May be an orofacial tic/spasm or may be some issue with the lip or the mouth itself or may reflect an underlying medical disorder - low calcium can present with facial numbness/spasms etc. So best to see a doctor as will need further detailed history, physical examination and may if cause unclear need tests.
