What does sonohysterogram (+/-echovist) mean? is it different from saline ultrasound? are they using contrast?

Echovist: is an ultrasound contrast agent. The procedure with saline and/or Echovist is essentially the same. However, due to technical differences in the way each appears under ultrasound visualization, saline is better for evaluating the uterine cavity, and Echovist is better for assessing tubal patency.
