The symptoms are: unlikely to be either from a stroke or heart attack but may be from an arthropathy. It would be reasonable to make an appointment for evaluation by a health care provider to obtain a definitive diagnosis and treatment.

Right arm pain: Pain in the right wrist would be an unusual presentation of a heart attack. A pinpoint stroke in the thalamus in the brain can present with pain but I feel is unlikely in your Case. Since you're experiencing some associated discomfort in the right shoulder, a nerve impingement in the neck is a possibility. Primary inflammation of the right wrist is another possibility. Unlikely: Given limited information available, heart attack and/or stroke are not likely, always possible otherwise. Your symptoms are not typical of above diagnoses and suggest rather arthritis flareup or neuropathy, carpal tunnel and such, specifically if there are numbness and/or tingling involved.
