Chest pain : Chest pain during exertion is often a sign of heart disease. It is not always easy to distinguish muscle pain, chest wall pain, or esophageal pain, from heart pain. The heart rate normally increases during exercise, but a heart rate of 200 (if that's correct) in a 61 year old is almost certainly an abnormal heart rhythm.
