Hives/spreading fast: If your baby has a sudden onset of raised hives/severely itchy rash all over the body that continues to spread quickly, it may be a sign of a severe allergic reaction. Also any purple or black looking spotted rash, especially associated with fever, vomiting, and ill-appearing child could be a sign of a life-threatening rash.

Sometimes not easy: However, if your child has no fever, is acting normally, playing, eating and sleeping normally, these are all good signs. If the rash is due to a serious infection, there are usually other symptoms. Rashes that do not blanch when you press on them are serious, other then that you should call your pediatrician and describe the rash and follow instructions.
