Many causes: A 26 year old man with chest pressure and pain, constant and with normal ECG and X-rays but improved with sleep. Causes could include psychogenic, esophageal dysmotility (diffuse esophageal spasm), esophageal pathology, aortic pathology, etc. At that age, serious conditions are unlikely but possible.
