Maximum Dose: By "hydromorphone contin" i asume you mean long acting hydromorphone or exalgo. There is no maximum dose of any pure mu opioid medication. That said going up and up and up on a drug with no meaningful improvement in pain or level of function makes no sense.

Co-analgesics: What is more important than "maximum"dose, is clinically effective dosing. Was the medication working then stopped working? Was it never effective? Small slow increases might be needed if a patient is becoming "tolerant" to the drug, or if the condition is worsening. If the med isn't working, switching narcotics or adding co-analgesic agents might be best.
