Maybe : But sounds more like airways problem, a reactive airway disease, (? bronchitis or asthma) can be the case, affected by upper airways inflammation, (nasal cavity and sinuses) denoted by the dark circles under his eyes. He may very well need an antibiotic and a bronchodilator, (a medicine to open up his lower airways) that would help his cough and shortness of breath. Kindly revisit his doctor
