Swelling? Arthritis : This could be arthritis or could be something else it's hard to clarify. This is something you need to check with your physician

Varies: A lot of potential causes, I would not hesitate being seen. Was there an injury? Common causes can include inflammatory conditions, conditions associated with a subtle deformity, a neuroma, stress injuries/fractures. A detailed exam and X-ray would go a long way in making the correct diagnosis.
