More testing: You are "spilling"'out protein and blood in your urine. A CT scan is helpful in ruling out major structural caused and large stone, the urine analysis ruled out infection, the cystoscope would evaluate for abnormalities of the bladder. Other considerations would be systemic problems such as a coagulopathy or inflammatory condition, lupus being an example. If all those tests neg, consider biopsy
