It depends: The over-the-counter HIV tests can have false positive results, but 40% is high. The antibody tests need to be confirmed by a western blot (looks more specifically at HIV antibodies) and perhaps a HIV viral load test to make sure someone doesn't have acute infection. It would be best to go to a doctor or depart of public health for more specific testing. More rapid tests will not be helpful.
