Yes: I recommend reconstruction surgery for acl i would prefer the cadaver graft.

Usually: Start with good pt program. Then it depends on your activity levels and whether or not you play sports. If the knee is unstable with normal activities then it needs to be done. Patellar tendon autograft or hamstring autograft recommended.
