Many possibilities : Muscle spasm , neck glands up due to a condition, reaction to something, cyst or lipoma among others. Treatment will be managing underlying cause. See your doctor for examination and if necessary tests

Swelling: I am not quite sure what you are asking, but there is something called a buffalo's hump which you can google to see images of. This is caused by an excess of cortisol in your system. Treatment is treating the underlying condition. Again, I am not sure even if this is what you are talking about.
