Infected cyst: Sounds like an infected cyst. These cysts are actually fairly common in the tissue behind the ear. If it is bothering him it can be removed fairly simply with a procedure done under local anesthesia.

Sebaceous cyst: Sounds like a sebaceous cyst. These usually need to be removed to get rid of the problem.
