Many things: I can list a number of things that might cause this from very benign to very concerning things as well. It is painful? Any symptoms otherwise? I'd advise that he be seen as soon as possible to ensure that something serious is not the case.

Get checked urgently: While gradual onset numbness and headache may be migraine if these are new symptoms in someone over 40 they warrant urgent medical assessment and consideration of more serious brain or neurological conditions.
