Otolaryngologist: A mass at the base of the tongue is of concern and an otolaryngologist should be involved. White matter changes in a brain ct scan or mri could be due to a demyelinating disease like multiple sclerosis and the neurologist is best qualified to evaluate and advise you about these findings.
