If both equally: effective, probably the better choice is the Tylenol/acetominophen. Ibuprofen, in higher doses, can adversely affect the kidneys. It is very important to protect the kidneys especially with high blood pressure. But Tylenol may not be as effective in relieving joint pain. Short term--4 to 5 days-- use of ibuprofen, doses not exceeding those listed on package, is acceptable. Splints also help:)

Probably Tylenol : Assuming there are no contraindications to Tylenol, it is probably safer for a patient with hypertension to try Tylenol first rather than NSAIDs such as ibuprofen. NSAIDs can cause fluid retention and elevation of blood pressure.
