"Lens" for US: Ultrasound works best when the bouncing of sound waves from the ultrasound is simple. For example, if you bounced a rubber ball off a flat wall, you could estimate how far the wall is away. Different materials conduct sound waves differently and the water in a full bladder helps the sound waves travel in a predictable way (bounce off the bladder wall) and puts structures where we can see them
