About 6 hrs: You should get some effects in about 6 hrs but the peak effect takes a day or two In general, I would advise against getting cortisone shots for a sinus infection especially for someone with anxiety & BP issues since nasal steroid likely works better with no serious side effects. If you have frequent sinusitis and allergies, best to consult an allergist.
