Over age 50-never!: The posterior tibial tendon supports the arch of the foot at the medial ankle. Athletes report pain and collapse of the arch with progressive flat footed deformity. Treatment involves immobilization in a boot for severe pain or supportive orthotics. Cortisone injections should be avoided. In athletes over 50, especially overweight females, it does not heal! Return to sports when pain subsides.
