What do you mean?: What do you mean by "spiritual?" this is not a term that is often used by mental health or medical practitioners when discussing the various causes of mental disorders. Usually we speak of "biopsychosocial" causes. But, to explain mental disorders we consider all aspects of a person, including one's religious or spiritual beliefs. Certainly, what one believes affects one's mental health.

Contributor Vs Cause: Spiritual issues, such as questioning your faith, “Catholic guilt,” etc, can certainly contribute to mental illness, or with delusions they can be the fictional foundation. However, spirituality or faith related concerns are not likely to be the sole cause of a mental illness. For example, if you have poor coping skills you may find yourself feeling anxious about questioning your faith.
