Concerning: One kidney smaller than the other could be suggestive of renovascular disease which could either be the cause or the effect of your high bp. See a nephrologist; there are specific drugs which should be used to control BP in your case, over others.

High BP small kidney: At 20 years of age, with very high BP and a small kidney, you need to be evaluated for a renal artery stenosis. You will need to have a or interventional radiologist inject dye into an artery and follow that into the renal arteries. I suspect that your small kidney has a narrowing of its renal artery that is causing your malignant bp. If you do have a narrowing, an agioplasty will cure your bp.
