Go to ER: You seem quite unwell. You may have a few different things happening together. It sounds like you have a varicocele in your left testes (which is swollen veins). The abdominal pain and lower back pain may mean you have with a kidney infection or possibly a hernia. You are constipated so the pressure during defecation would make matters worse. There may be an obstruction somewhere as well. Go to ER
