You can't: If the pain started in the middle and then went down and to the right, and if you have any nausea, vomiting, fever, diarrhea or constipation, it is slightly more likely to be appendix. The only way to try to know for sure is to be examined. Of course if it goes away, it is unlikely to have been your appendix.
