Smell: What is wrong is that your friend is not spending time with you properly. I am sure you like your friend very much but you must carefully explain to him what has been bothering you.

Have him seen: get him to a physician. Uncircumcised males can get an accumulation of skin/bacterial called smegma. Infection must be ruled out. Politely mention this to him.
