Cause of pain: the solution to this problem depends solely on the cause. Gas can cause intense pain, and is indeed "air", but some abdominal organs will radiate pain to the back or the shoulder when they are infected. Good idea to see a doctor, who may order labs or imaging in addition to getting a really thorough history and physical exam.

Back pain : Probably muscle spasm if pain does not radiate to thigh or legs. Back strengthening exercise trial for 6 weeks.
