Urethritis: May be bacterial, inflammation or related to a urinary tract infection I advise a full GYN exam, urine analysis and cultures.

Swelling: If the area looks red, it may be caused by candida (yeast) and can sometimes be treated with topical antifungal creams. See a doctor soon.
