My doctor wants to charge me both coinsurance & copay instead of just copayment. even when it states my copay on my card is 25$. she is charging more?

Depends on policy: It all depends on one's insurance policy. That means it depends on what plan you chose or you bought. Many plans do have both copay and coinsurance. Check with your insurance company if you don't have the plan benefits (which should be 2-4 pages long and would never fit on an insurance card).
