1st visit w/your doc: Before your 1st visit w/new doc, get prepared by listing all your medical problems, surgeries, allergies & medications. Immunizations, too. If you have a number of concerns, put them in order that bothers you most. We may re-order them but at least we know what's important to you. Plus sign release of info at all other docs so they can forward your records to new doc to review.
