Diarrhea: Severe diarrhea may be due to a viral intestinal infection but should show gradual improvement over several days without any specific treatment. If diarrhea remains severe and unrelenting,stool culture is indicated to check for pathogenic bacteria or parasites which do require specific treatment. I recommend an evaluation in the hospital emergency room in view of your symptoms with flank pain.

Diarrhea: Would go to ER as you may be dehydrated from much diarrhea. May be E coli diarrhea which is severe, can have complications and is treated symptomatically. Would try probiotic to repelnish bacteria, , keep hydrated with water/ gatorade avoid diary, solids. Pain may be colitis / inflammation from diarrhea. Go to ER 4 days is a lot of diarrhea. DO NOT take immodium
