Internal Medicine 40 years experience
RLQ pain: May be ileitis , even chronic subclinical appendicitis http://www.ncbi.nlm.nih.gov/pmc/articles/PMC4416293/, internal hernia, , mass in pelvis/ abdomen( from ovary) , small bowel or even stone ( though that usually is constant) .Would follow up with doc and consider CT of abdomen/ pelvis with and without contrast .
