?metatarsalgia: See a podiatrist to get this addressed, as you may have already been diagnosed with Morton's metatarsalgia. Plantar fasciitis and several rheumatological conditions could be playing a role here, and would advise blood studies if answer is elusive.

What size ball....: Issues of "chronic" pain cannot be easily resolved without first discovering the history and true nature of the pain and it's chronicity. The ball of the foot means different things to different people. Does it involve just the first metatarsal or all five. A thorough back and forth type consultation would be the best way get to the nitty gritty of your pain.
