Many possibilities.: Pelvic floor physical therapy combined with topical anesthetic or a tricyclic antidepressant is a great first line therapy. Their are other medications that can be used. Often trial and error. Local injection of the area can be helpful. Their are other solutions besides dilators. Can be a complex issue. Hope this helps.
