TWO THINGS: I'm assuming no injury/trauma. First, is it the very early signs of a pinched nerve supplying the second and third toes; it's called a neuroma, most commonly affecting the 3rd and 4th toes. Second, if diabetic, is it the very, very early signs of diabetic neuropathy? Those are my thoughts. Sincerely, Dr. Latva

Long 2nd & 3rd toes.: I have only seen this when the foot exhibits a very specific toe length pattern. Typically, when the 2nd and 3rd toes are longer than the others, the increased length can be a problem with sock sizing and shoes. Walking causes the long toes to flex harder against the bottom of the shoes, especially if hampered by sock pressure. This is not easily solved without a visual examination.
