My first u/s, the baby was 12w 4d. could my lmp have been implantation making baby 14 weeks instead and u/s tech not notice or question the size?

Yes: U/S dating is very accurate in the first trimester. If there is a difference > 1 week between your LMP due date and the U/S due date, the U/S is more accurate. Bleeding in early pregnancy can easily be confused with a period. Best wishes!

Yes, but unlikely: Ultrasound dating estimates are pretty accurate (plus/minus one week). If a woman has very regular periods, her LMP was likely her regular period. Finally, 12 weeks 4 days is close to being 14 weeks 0 days, when looking at the RANGE of the estimated date from ultrasound scanning. Thus, think no more of it... and consider the dates one and the same... as all dates are just estimates.
