Orthopedic or PMR: Well first make sure your internist has checked your electrolytes and taken a good history to makes sure there is nothing making you at risk for cramps. Then either a physiatrist or orthopedic doctor could rule out that whether the problem is coming from your back, (such as lumbar stenosis). Physical therapy can be helpful for many conditions, so it can not hurt to try as it could bring relief.
