Depends on cause: Sciatica can occur at lumbar root level, but a piriformis syndrome can also cause symptoms. Maybe best to see physiatrist, and obtain a course of physical therapy. Occasional injections, especially epidural steroids may be stabilizing. Surgery is an option of "last resort"
