Spine Pain Options: This chronic pain in the distribution as you suggested is the result of an irritated nerve or facet joints or other injury typically in the lumbar spine (low back) which are caused by herniated disks, spinal stenosis or degenerative disc disease, etc requiring further evaluation by a spine specialist and may be candidate for facet injections/radiofrequency ablation and epidural steroid injection.

Needs Evaluation: She needs to be evaluated with up to date testing and a physical examination. Not sure if she has previously tried epidurals and physical therapy, but both typically help. If she has failed to respond to these treatments then surgery may be her next step. Physical therapy and other conservative measures, including medication management, are considered first line treatment. If these fail, your physician may recommend epidural steroid injections.

Great Question: Really depends on how long she has had her sx., are there neurological deficits such as weakness, numbness etc. And what does imaging (MRI) show. Surgery is the tx. Of absolute last resort unless there are serious neurological findings. Pain alone does not mean surgery is indicated. Many treatments such as heat, ice, rest, physical therapy and anti-inflammatories often really help these sx.
