Depo shot: You are supposed to get depo progesterone shots every 3 months, not 1 per year! It does not prevent ovulation. It prevents implantation so the little critter never gets the chance to settle down and form a connection to you(a placenta) and grow.

Too long ago: A Depo-Provera shot lasts 3 months, which means a woman is no longer protected against pregnancy if it has been longer than 3 months since the last shot. May 2015 was eleven months ago. Thus, if not wanting to get pregnant, one must stop having sex until a reliable form of birth control (contraception) is started.
