You need further: evaluation. Testis could be to intermittent torsion, twisting causing ischemia, lack of blood flow intermittently. If no masses, that's a good sign; lower abdominal pain could be gas; yet,..a gastroenterologist may want to do a colonoscopy. An inguinal hernia cannot be ruled out either, a gen. surgeon evaluation
