Not a STD: Gonorrhea is only one of the possible causes of epididymitis; chlamydia is another but I'm not sure all the ways of testing are reliable. Some gram-negative rod bacteria may have found their way up your vas deferens to your prostate. The STD check is more about certifying you safe for others -- and even then, I'd urge you to use barrier protection. A long course of antibiotics usually clears it.

Many possibilities: 1) Make sure diagnosis is correct; not all testicular pain is due to epididymitis. 2) Testing misses some gonorrhea and chlamydia, especially with urine tests. 3) Although at your age STDs are the most common causes, other infections can cause it. 4) Epididymitis can result from reaction to certain drugs. 5) Some cases just have no apparent cause. Your sex partners still need STD testing.
