Possibly...: There are many reasons for subungual toe pain (pain under the nails). One potential cause easily diagnosed with x-ray is an exostosis, or boney projection, pressing on the underside of the nail plate.

Bone spur: It is fairly common to have a small bone spur under the great toenails. Shoe pressure will then cause pain. Have a podiatrist evaluate it. X-rays will confirm a spur and removal is fairly simple.
