Injury? Fissure?: If the anal sex was traumatic, you may simply have an injury. Or maybe an anal fissure. Perhaps some chance of herpes, depending on risk factors. Syphilis and other STDs are less likely, and of course also depend on STD risks. See a doctor to check it out. Good luck!
