Mgt: I would suggest a pulse oximetry, Blood Pressure measurement and cardiac evaluation by your PCP. Sometimes these symptoms are related to cardiac causes. A virtual appointment is also available.

Not necessarily: The head of a kid that age is a relatively large part of their body surface area and an easy portal to discard heat/sweat after running around. If the kid is robust, engaging in vigorous play,feeding well and growing normally I would have little concern.If he has only brief active periods followed by prolonged rest, I would want him evaluated.
